Extramurale veneuze invasie (EMVI) op baseline-MRI is geassocieerd met slechte prognose in patiënten met lokaal-gevorderd rectumcarcinoom (LARC). Een retrospectieve studie van Memorial Sloan Kettering Cancer Center (New York) heeft de associatie van persistente EMVI na totale neoadjuvante therapie (chemoradiotherapie en systemische chemotherapie) voor LARC met de overleving geïnventariseerd. De studie includeerde 175 LARC-patiënten die tussen begin 2010 en eind 2017 TNT en totale mesorectale excisie ondergingen. EMVI-positiviteit op baseline-MRI was ook na TNT geassocieerd met slechtere DFS en OS. EMVI-regressie op zowel post-TNT MRI en chirurgische pathologie was geassocieerd met betere DFS (HR 0,17; 95%-bti 0,04-0,64) en OS (0,11; 0,02-0,68). Regressie op alleen pathologie, ongeacht MRI EMVI-status, was geassocieerd met vergelijkbare verbeteringen in overleving.

De onderzoekers concluderen dat baseline EMVI ook na TNT geassocieerd was met slechtere prognose. EMVI-regressie op chirurgische pathologie was geassocieerd met betere overleving, ongeacht de post-TNT MRI EMVI-status.
