Er is
behoefte aan prognostische tools voor het schatten van het risico van recidief
na resectie met curatieve intentie van tot de lever beperkt metastatisch
colorectaalcarcinoom (LL-mCRC). In gelokaliseerd CRC is circulerend tumor DNA
(ctDNA) een veelbelovende marker als surrogaat van postchirurgische minimale
residuele ziekte (MRD). Een retrospectieve multicenterstudie in Italië heeft de
waarde van postoperatief ctDNA als marker van MRD na radicale resectie van
LL-mCRC geïnventariseerd. Dr. Federica Marmorino (Universiteit van Pisa) en
collega’s publiceren de studie in JCO Precision Oncology.1
De studie
includeerde 76 patiënten met LL-mCRC. In 39 patiënten (51%) werd postoperatief
ctDNA gedetecteerd. Na mediaan 77 maanden follow-up was recidief gezien in 33
van 39 ctDNA-positieve patiënten versus 20 van 37 ctDNA-negatieve patiënten,
met mediane recidiefvrije overleving van 12,7 versus 27,4 maanden (HR 2,09;
p=0,008). In een multivariabel model met correctie voor andere prognostische
covariaten bleef de associatie significant (p=0,046). Er was een trend van
kortere overall survival in de groep ctDNA-positieve patiënten (HR 1,65;
p=0,183).
De
onderzoekers concluderen dat detectie van postoperatief ctDNA na resectie van
LL-mCRC een onafhankelijke negatieve prognostische marker voor RFS was.

relapse-free survival and a trend of shorter overall survival.
