Het
standaard thorax-radiotherapie (RT)-schema voor beperkt-stadium kleincellig
longcarcinoom (LS-SCLC) is 45 Gy tweemaal daags. De multicenter fase 3-studie
CALGB 30160/RTOG 0538 heeft deze dosering vergeleken met 70 Gy eenmaal daags;
in beide groepen gecombineerd met vier cycli chemotherapie. Prof. Jeffrey Bogart (State University of New
York Upstate Medical University, New York) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 313 patiënten in de tweemaal-daagse arm en 325 in de eenmaal-daagse
arm. De mediane follow-up was 4,7 jaar. De mediane overall survival (primair eindpunt) was 28,5 maanden in de
tweemaal-daagse arm versus 30,1 maanden in de eenmaals-daagse arm, met
vijf-jaars OS-percentage 29% versus 32% (p=0,594). De behandeling werd
verdragen, en de frequentie van ernstige adverse events verschilde niet significant
tussen de armen.
De
onderzoekers concluderen dat 45-Gy tweemaal daags het standaard RT-schema
blijft, maar dat de studie wel robuuste informatie levert die van belang kan
zijn voor het kiezen van het optimale schema voor LS-SCLC.

versus 45-Gy twice-daily for LS-SCLC. Median survival was 28.5 months for the
twice-daily treatment versus 30.1 months for the once-daily treatment, with
5-year OS or 29% and 32%, respectively. Treatment was tolerable, and the
frequency of severe adverse events was similar for both arms.
