LY3300054 (lodapolimab) is een nieuwe PD-L1 remmer. De multinationale fase 1a-1b studie PACT onderzocht de veiligheid en klinische activiteit van LY3300054 als monotherapie of in combinatie met targeted therapies voor gevorderde refractaire solide tumoren. Het fase 1a-gedeelte van de studie includeerde patiënten die oplopende doseringen LY3300054 kregen als monotherapie (n=15) of in combinatie met ramucirumab (n=10), abemaciclib (n=24), of merestinib (n=12). Het fase 1b-gedeelte includeerde 8 patiënten met melanoom in de monotherapie-arm en 12 patiënten met pancreascarcinoom in de combinatie-arm met merestinib. In 4 patiënten werd een veertien dagen durende run-in met abemaciclib geëvalueerd. De combinatiebehandelingen werden concurrent gegeven vanaf dag één van elke cyclus. Primaire eindpunten waren doseringslimiterende toxiciteiten en veiligheid.

Treatment-related adverse events waren vermoeidheid (13% van de patiënten) en misselijkheid (13%) in de monotherapie-arm, hypothyreoïdie (30%) in de ramucirumab-arm, diarree (54%) in de abemaciclib-arm, en misselijkheid (25%) in de merestinib-arm. In de abemaciclib run-in arm werd in 3 van 4 patiënten doseringslimiterende hepatoxiciteit gezien; in de concurrent abemaciclib-arm was er geen graad 3 of hoger hepatotoxiciteit. In elk van de vier armen was er één patiënt met partiële respons die 7 maanden of langer aanhield.

De onderzoekers concluderen dat LY3300054 als monotherapie of in combinatie met ramucirumab, abemaciclib, of merestinib verdragen werd zonder onverwachte toxiciteit, en dat duurzaam klinisch profijt werd gezien met alle regimes. Run-in abemaciclib voorafgaand aan combinatie met LY3300054 was niet feasible vanwege hepatotoxiciteit.
