Bendamustine plus rituximab (BR) is een veelgebruikte frontline therapie voor folliculair lymfoom (FL). ⁹⁰Y-Ibritimumab tiuxetan (⁹⁰YIT) is een CD20-gericht radiotherapeutisch antilichaam. De prospectieve multicenter fase 2-studie Fol-BRITe onderzocht de veiligheid en werkzaamheid van inductie met BR gevolgd door consolidatie met ⁹⁰YIT voor niet-eerder behandeld FL. Dr. Frederick Lansigan (Dartmouth-Hitchcock Medical Center, Lebanon NH) en collega’s publiceren de studie online in Clinical Cancer Research.¹

De studie includeerde 39 volwassen patiënten die behandeling nodig hadden voor chemotherapie-naïef stadium II tot en met IV FL. Tweeëndertig patiënten (82%) hadden intermediair- of hoog-risico ziekte, en zes (15%) hadden graad 3a-ziekte. De patiënten kregen initieel rituximab gevolgd door vier cycli BR, en zes tot twaalf weken na BR consolidatie met ⁹⁰YIT in patiënten met tenminste partiële respons. De inductie resulteerde in respons in 37 patiënten (94,8%) onder wie 30 met complete respons/niet-bevestigde complete respons (CR/CRu; 77%). Consolidatie met ⁹⁰YIT resulteerde in conversie van partiële respons (PR) naar CR/CRu in 81%. Na 45 maanden follow-up was nog 71% (95%-bti 0,57-0,89) van de patiënten progressievrij.

De onderzoekers concluderen dat het regime van vier inductiecycli BR gevolgd door ⁹⁰YIT-consolidatie resulteerde in hoge percentages duurzame respons. De consolidatie resulteerde in hoge percentages conversie van PR naar CR/CRu.

Summary: The multicenter phase 2 study Fol-BRITe found that in untreated follicular lymphoma four cycles of bendamustine-rituximab followed by consolidation with ⁹⁰Y-ibritumomab tiuxetan resulted in high rates of durable responses. In addition, the consolidation resulted in high rates of conversion of partial response to complete response.
