Er zijn
weinig systemische behandelopties voor patiënten met hersenmetastasen van
niet-kleincellig longcarcinoom (NSCLC-BMs). De multicenter fase 2-studie
CAP-BRAIN in China heeft de eerstelijns combinatie van camrelizumab,
pemetrexed, en carboplatine voor patiënten met niet-squameus NSCLC-BMs
geëvalueerd. Dr. Likun Chen (Sun Yat-sen University Cancer Center, Guangzhou)
en collega’s publiceren de studie in het Journal of Thoracic
Oncology.1
CAP-BRAIN
werd uitgevoerd in zeven centra. De studie includeerde veertig patiënten met
tenminste één post-baseline tumorbepaling. Het primaire eindpunt was het
percentage patiënten met intracraniële objectieve respons (iORR). De mediane
follow-up was 12,5 maanden. De iORR was 52,5% (95%-bti 36,1-68,5), en de
extracraniële ORR was 47,5% (31,5-63,9). De mediane intracraniële
progressievrije overleving was 7,6 maanden (95%-bti 4,6-NR), de mediane overall
PFS was 7,4 maanden (4,4-NR), en de mediane overall
survival was 21,0 maanden (15,9-NR). Eén patiënt overleed aan met de
behandeling samenhangende pneumonie. Er was een trend van verbetering van
cognitief functioneren en kwaliteit van leven.
De
onderzoekers concluderen dat de eerstelijns combinatie van camrelizumab,
pemetrexed, en carboplatine activiteit had voor nsqNSCLC met BMs, met een manageable toxiciteit.
