De waarde
van de combinatie van PARP-remming plus chemotherapie voor extensief-stadium
kleincellig longcarcinoom (ES-SCLC) is niet duidelijk. Een multinationale
gerandomiseerde fase 2-studie heeft de combinatie van veliparib met
carboplatine-etoposide chemotherapie voor niet-eerder behandeld ES-SCLC
geëvalueerd. Dr. Lauren Byers

De studie
includeerde 181 patiënten, die 1:1:1 werden gerandomiseerd naar zes cycli
veliparib plus chemotherapie gevolgd door veliparib onderhoud (‘veliparib
throughout’ groep), veliparib plus chemotherapie gevolgd door placebo onderhoud
(‘veliparib combination only’ groep), of placebo plus chemotherapie gevolgd
door placebo onderhoud (‘controlegroep’). Het primaire eindpunt was
progressievrije overleving met veliparib throughout versus controle.
De PFS was
beter met veliparib throughout dan met controle (HR 0,67; 80%-bti 0,50-0,88).
Onder patiënten met SLFN11-positieve ziekte was de PFS-HR 0,6 (80%-bti
0,36-0,97). De mediane overall survival was niet beter met veliparib throughout
dan met controle (HR 1,43; 80%-bti 1,09-1,88). Graad 3 of 4 adverse events werden gezien in 82% van
de patiënten in de veliparib throughout groep, 88% in de combination only
groep, en 68% in de controlegroep.
De
onderzoekers concluderen dat veliparib in combinatie met platina-chemotherapie
gevolgd door veliparib onderhoud voor niet-eerder behandeld ES-SCLC resulteerde
in verbetering van de PFS maar niet OS vergeleken met alleen
platina-chemotherapi.
