Fase 3-studie van adjuvant S-1 met of zonder docetaxel voor stadium III maagcarcinoom


Prof. Kazuhiro YoshidaIn Aziatische landen is S-1 een standaard postoperatieve chemotherapie na resectie voor stadium III maagcarcinoom (GC). De Japanse multicenter fase 3-studie JACCRO GC-07 onderzocht de waarde van toevoegen van docetaxel aan S-1 in deze setting. Prof. Kazuhiro Yoshida (Universiteit van Gifu) en collega’s publiceren een interimanalyse van de studie online in het Journal of Clinical Oncology.1

De studie randomiseerde 915 patiënten na resectie voor stadium III GC naar S-1 plus docetaxel of alleen S1. Het primaire eindpunt van de studie was recidiefvrije overleving. De interimanalyse werd uitgevoerd na 216 RFS-gebeurtenissen (mediaan 12,5 maanden follow-up). De figuur (panel A: alle patiënten; panel B: patiënten met stadium IIIA; panel C: IIIb; panel D: IIIC) laat zien dat de recidiefvrije overleving beter was in de combinatie-arm dan in de alleen S-1 arm. Voor alle patiënten tezamen was de drie-jaars RFS HR 0,632 (p<0,001). De incidentie van graad 3 en hoger adverse events was hoger in de combinatie-arm, maar alle AEs waren manageable.

De onderzoekers concluderen dat toevoegen van docetaxel aan adjuvant S-1 na R0-resectie voor stadium III GC resulteerde in verlenging van de RFS.

Summary: The Japanese phase 3 study JACCRO GC-07 randomized patients after resection for stage III gastric cancer to adjuvant S-1 or S-1 plus docetaxel. The relapse-free survival was better in the combination arm than in de S-1 monotherapy arm (A: all patients; B: IIIA patients; C: IIIB patients; D: IIIC patients). The adverse events were manageable.
