Er is geen
duidelijkheid over de waarde van preoperatieve lymfoscintigrafie (LSG) voor het
verbeteren van de accuratesse van stadiëring van de ziekte in patiënten die
schildwachtklierbiopsie (SLNB) ondergaan voor vroeg stadium mammacarcinoom. De prospectieve
Duits/Zwitserse multicenter fase 3-studie SenSzi randomiseerde patiënten naar
SNLB met of zonder bij de chirurg bekend LSG-resultaat. Prof. Sherko Kümmel
(Kliniken Esse-Mitte) en collega’s publiceren de studie online in het Journal of Clinical
Oncology.1
SenSzi werd
uitgevoerd in 23 centra in Duitsland en Zwitserland. De studie includeerde 1198
patiënten met cN0 vroeg-stadium mammacarcinoom of extensief/hooggradig DCIS. De
patiënten werden 1:1 gerandomiseerd. Het primaire eindpunt van de studie was
het per patiënt gemiddeld aantal histologisch gedeteceerde SLNs. De vooraf
gedefinieerd niet-inferioriteitsmarge was 10%. Gemodificeerde ITT-analyse kwam
tot gemiddeld aantal histologisch gedetecteerde SLNs van 2,21 met LSG versus
2,26 zonder LSG, waarmee niet-inferioriteit van achterwege laten van
preoperatieve LSG was aangetoond. Er waren evenmin statistisch significante
verschillen voor de secundaire eindpunten percentages klierpositieve patiënten
en axillaire lymfeklierdissectie.
De
onderzoekers concluderen dat kennis van de LSG-resultaten geen invloed heeft op
de effectiviteit van SLNB. Achterwege laten van LSG resulteert in tijdwinst en
kostenbesparing.

prospective phase 3 study SenSzi in
Germany and Switzerland randomized patients with early breast cancer to sentinel
lymph node biopsy with versus without surgeon’s knowledge of the findings of
preoperative lymphoscintigraphy. The study found noninferiority of omitting
preoperative LSG. SLNB without LSG will speed up the preoperative workflow and
reduce cost.
