De optimale
behandelstrategie voor niet-eerder behandeld metastatisch colorectaalcarcinoom
is niet duidelijk. De Duitse multicenter non-inferioriteits fase 3-studie
XELAVIRI randomiseerde mCRC-patiënten naar eerstelijns behandeling met een
fluoropyrimidine plus bevacizumab, gevolgd door toevoeging van irinotecan bij
eerste progressie (arm A), of upfront fluoropyrimidine plus irinotecan plus
bevacizumab (arm B). Dr. Dominik Modest (Ludwig-Maximilians Universität,
München) en collega’s publiceren de studie online in het Journal of Clinical
Oncology.1
De studie
randomiseerde 212 patiënten naar arm A en 209 naar arm B. De mediane leeftijd
in de beide armen was 71 respectievelijk 69 jaar. Het primaire eindpunt van de
studie was time to failure of the strategy
(TFS), met als non-inferioriteitsmarge een HR van 0,8. Non-inferioriteit werd
niet aangetoond, met voor arm B versus arm A HR 0,86 (90%-bti 0,73 tot 1,02).
Patiënten met RAS/BRAF-wildtype
tumoren hadden significant betere TFS in arm B dan in arm A (HR 0,61; p=0,005),
terwijl er voor patiënten met RAS-mutant
tumoren geen significant TFS-verschil was tussen beide armen (HR 1,09; p=0,58).
De p voor interactie tussen studie-arm en RAS-status
was 0,03.
De
onderzoekers concluderen dat voor TFS non-inferioriteit van sequentiële
therapie vergeleken met initiële combinatietherapie niet is aangetoond. De
upfront combinatietherapie was significant superieur voor RAS/BRAF-wildtype tumoren, maar er was geen TFS-verschil tussen
beide behandelingen voor RAS-mutant tumoren.
