De multicenter
fase 3-studie NCT01872962 in China randomiseerde patiënten met locoregionaal-gevorderd
nasofarynxcarcinoom (NPC) naar gemcitabine-cisplatine inductiechemotherapie
gevolgd door concurrente chemoradiotherapie (n=242) of alleen concurrente
radiotherapie (n=238). In 2019 is gepubliceerd dat met mediaan 42,7 maanden follow-up het drie-jaars
recidiefvrije overlevingspercentage significant hoger was in de
inductietherapiegroep dan in de controlegroep (85,3% versus 76,5%; HR 0,51;

resultaten van de studie.1
De mediane
follow-up op het moment van de nu gepubliceerde analyse was 69,8 maanden. Het
vijf-jaars OS-percentage was 87,9% in de inductiechemotherapie groep versus
78,8% in de controlegroep (HR 0,51; p=0,001). Het percentage patiënten met late
toxiciteit verschilde niet significant tussen de twee groepen (graad 3 of
hoger: 11,3% versus 11,4%). De diepte van de tumorrespons op
inductiechemotherapie was significant gecorreleerd met de OS: vijf-jaars
OS-percentages waren 100% in de groep patiënten met complete respons, 88,4% in
de groep patiënten met partiële respons, en 61,5% in de groep patiënten met
stabiele of progressieve ziekte (p=0,005). Patiënten met laag pretreatment
celvrije Epstein-Barr virus DNA-gehalte (minder dan 4000 kopieën per ml) hadden
geen baat bij inductiechemotherapie (vijf-jaars OS-percentage 90,6% versus
91,4%; p=0,77).
De
onderzoekers concluderen dat inductiechemotherapie voorafgaand aan concurrente
chemoradiotherapie voor locoregionaal-gevorderd NPC geassocieerd was met
significant verbeterde overleving, zonder verergering van late toxiciteit.
Tumorrespons op inductiechemotherapie en pretreatment EBV-DNA kunnen wellicht
een rol spelen in het personaliseren van de behandeling.
