Er zijn
aanwijzingen voor een associatie tussen alcoholgebruik en het risico van biliary tract cancer (BTC). Een
prospectieve studie in Zuid-Korea heeft de impact van glycemische status op de
associatie tussen licht-tot-matige alcoholconsumptie en het BTC-risico
geïnventariseerd. Prof. Joon Oh Park (Samsung Medisch Centrum, Seoel) en
collega’s publiceren de studie in het Journal of Clinical Oncology.1
De nationwide studie includeerde 9.520.629
personen die in 2009 ouder waren dan negentien jaar, geen geschiedenis van
maligniteiten hadden, en deelnamen aan een gezondheids-screening waarbij ze
informatie gaven over onder meer hun alcoholgebruik. Ze werden tot eind 2018 gevolgd
voor ontwikkeling van BTC, inclusief cholangiocarcinoom (CCA) en
galblaascarcinoom (GBC). Tijdens 78,3 miljoen persoonsjaren follow-up werd BTC
vastgesteld in 21.079 deelnemers. Licht-tot-matig alcoholgebruik was
geassocieerd met verhoogd CCA-risico in deelnemers met prediabetes (aHR 1,20;
95%-bti 1,13-1,28) en deelnemers met diabetes (1,58; 1,47-1,69) en met verhoogd
GBC-risico in deelnemers met prediabetes (1,18; 1,07-1,31) en in deelnemers met
diabetes (1,45; 1,28-1,64). In normoglycemische deelnemers was licht-tot-matige
alcoholconsumptie niet geassocieerd met BTC-risico. Vergelijkbare associaties
werden gezien in afzonderlijke analyses van intrahepatisch en extrahepatisch
CCA. Zwaar alcoholgebruik was ook in normoglycemische deelnemers geassocieerd
met verhoogd BTC-risico.
De
onderzoekers concluderen dat in personen met prediabetes of diabetes licht-tot-matige
alcoholconsumptie geassocieerd was met verhoogd BTC-risico.

Summary: A nationwide prospective cohort study in South Korea (78.3 million person-years of follow-up) investigated the impact of glycemic status on the association
between light-to moderate alcohol consumption and the risk of biliary tract
cancer (BTC). Light-to-moderate alcohol consumption was associated with
increased BTC risk in individuals with prediabetes and diabetes, but not in
normoglycemic participants.
