Ipilimumab met of zonder anti-PD-1 therapie voor anti-PD-(L)1 monotherapie resistent metastatisch melanoom


Prof. Georgina LongAnti-PD-(L)1 therapie resulteert in lange-termijn ziektecontrole in ongeveer 30% van de patiënten met metastatisch melanoom. De overige patiënten zijn resistent en hebben verdere behandeling nodig. Een multinationale retrospectieve studie heeft ipilimumab voor deze patiënten vergeleken met ipilimumab plus anti-PD-1 therapie (pembrolizumab of nivolumab). Prof. Georgina Long (Melanoma Institute Australia, Sydney) en collega’s publiceren de studie in The Lancet Oncology.1

De studie, uitgevoerd in vijftien melanoomcentra op drie continenten, includeerde 355 volwassen patiënten met niet-resectabel stadium III of IV melanoom na falen van anti-PD-(L)1 therapie (nivolumab, pembrolizumab, of atezolizumab). Onder deze patiënten waren 162 die vervolgens ipilimumab monotherapie kregen (monotherapiegroep) en 193 die ipilimumab plus pembrolizumab of nivolumab kregen (combinatietherapiegroep). De tumorrespons werd iedere drie maanden bepaald met CT of PET-CT. Eindpunten van de studie waren objective response rate, progressievrije overleving, overall survival, en veiligheid.

De mediane follow-up was 22,1 maanden .In de combinatietherapiegroep versus de monotherapiegroep was de ORR 31% versus 13% (p<0,0001), de mediane PFS 3,0 maanden versus 2,6 maanden (HR 0,69; p=0,0019), en de mediane OS 20,4 maanden versus 8,8 maanden (HR 0,50; p<0,0001). Het percentage patiënten met graad 3 tot en met 5 adverse events verschilde niet significant tussen de groepen (31% versus 33%). Eén patiënt in de monotherapiegroep overleed 26 dagen na de laatste behandeling aan colonperforatie.

De onderzoekers concluderen dat ipilimumab plus anti-PD-1 resulteerde in betere uitkomsten dan ipilimumab monotherapie voor metastatisch melanoom dat resistent was tegen anti-PD-(L)-1 therapie.

Summary: A multinational retrospective study compared ipilimumab alone versus ipilimumab plus anti-PD-1 therapy (pembrolizumab or nivolumab) for metastatic melanoma that was resistant to anti-PD-(L)1 monotherapy. The ORR, PFS, and OS were better in the combination group. Grade 3-5 adverse events rates were similar.
