CD19-gerichte
CAR T-celtherapie resulteert in hoge percentages patiënten met respons onder
kinderen en jongvolwassenen (CAYAs; tot en met leeftijd veertig jaar) met B-cel
ALL, maar in veel patiënten wordt recidief gezien. De rol van allogene
stamceltransplantatie na de CD19-CAR T-celtherapie is tot op heden niet
onderzocht. Een multicenter fase 1-studie in de Verenigde Staten heeft
lange-termijn uitkomsten van CD19-CAR T-celtherapie voor recidiverend of
refractair B-ALL in CAYAs geïnventariseerd. Prof. Crystal Mackall (National
Cancer Institute, Bethesda MD) en collega’s publiceren de studie in het Journal
of Clinical Oncology.1
De studie
includeerde vijftig CAYAs met R/R B-ALL. De mediane leeftijd was 13,5 jaar
(range 4,3-30,4). CD19.28ζ-CAR T-celtherapie resulteerde in complete remissie in 31
patiënten (62%) onder wie 28 (90,3%) MRD-negatief waren. Met mediane follow-up
van 4,8 jaar was de mediane overall
survival 10,5 maanden (95%-bti 6,3-29,2). Eenentwintig van 28 MRD-negatieve
patiënten (75%) ondergingen alloHSCT. De mediane OS in deze groep was 70,2
maanden (95%-bti 10,4-NE). De cumulatieve incidentie van relapse 24 maanden na
alloHSCT was 9,5% (95%-bti 1,5-26,8) en de vijf-jaars gebeurtenisvrije
overleving na alloHSCT was 61,9% (95%-bti 38,1-78,8).
De
onderzoekers concluderen dat alloHSCT na CD19-CAR T-celtherapie resulteerde in
duurzame ziektecontrole in een aanzienlijk percentage van de CAYAs met R/R
B-ALL.

B-ALL. CAR T-cell therapy followed by allogeneic hematopoietic stem-cell
transplantation mediated durable disease control in a sizable fraction of CAYAs
with relapsed or refractory B-ALL.
