Multicenter fase 1-2 studie van Ponatinib-FLAG-IDA voor CML in blastenfase

Dr. Graham McIlroyDe prognose van patiënten met CML in blastenfase (BP) is slecht. Lange-termijn overleving is alleen mogelijk in geval van bereiken van een tweede chronische fase, gevolgd door allogene hematopoïetische stamceltransplantatie (alloHSCT). De multicenter fase 1-2 MATCHPOINT studie heeft de combinatie van de TKI ponatinib met fludarabine, cytarabine, G-CSF, en idarubicine (FLAG-IDA) voor CML in blastenfase geëvalueerd. Dr. Graham McIlroy (University of Birmingham, UK) presenteert de studie op de Annual Meeting van ASH in Atlanta.1 De studie is ook gepubliceerd in The Lancet Haematology.2



MATCHPOINT werd uitgevoerd in acht centra in het Verenigd Koninkrijk. De studie includeerde twaalf mannen en vijf vrouwen met CML-BP. De patiënten kregen ten hoogste twee cycli Ponatinib-FLAG-IDA. De mediane follow-up was 41 maanden (IQR 36-48). Onder de zestien evalueerbare patiënten bereikten elf (69%) een tweede chronische fase na één cyclus behandeling. Vier patiënten (25%) hadden doserings-limiterende toxiciteit. Twaalf patiënten (71%) ondergingen alloHSCT, van wie zeven nog in leven zijn mediaan 36 maanden na de transplantatie. De mediane overall survival van het gehele cohort was twaalf maanden.

De onderzoekers concluderen dat de combinatie Ponatinib-FLAG-IDA een tweede chronische fase kan induceren in patiënten met CML-BP.

Summary: The multicenter phase 1-2 MATCHPOINT study in the UK found that the combination of the TKI ponatinib with fludarabine, cytarabine, granulocyte colony-stimulating factor, and idarubincine (FLAG-IDA) can induce second chronic phase in patients with blast-phase CML, representing an active salvage therapy to bridge to alloHSCT.
