In 2015 werd
de Franse fase 2-studie THERAPY gepubliceerd, die liet zien dat de combinatie
cetuximab plus trastuzumab na progressie van pancreascarcinoom op gemcitabine resulteerde
in stabilisatie van de ziekte in 27% van de patiënten. De Franse multicenter fase
2-studie GATE 1 heeft nu de combinatie van gemcitabine, trastuzumab en
erlotinib geëvalueerd als eerstelijns behandeling voor metastatisch pancreasadenocarcinoom.

De studie
includeerde 63 patiënten (37 mannen en 26 vrouwen, mediane leeftijd 62 jaar;
range 35-77), onder wie 62 evalueerbaar waren voor toxiciteit en 59 voor
werkzaamheid. Het primaire eindpunt was disease
control rate; secundaire eindpunten waren progressievrije overleving, overall survival, en toxiciteit. De
mediane duur van de behandeling met de triplet-combinatie was 16,1 weken (range
2,1-61). Partiële respons werd gezien in 11 patiënten en ziektecontrole in 33,
voor een DCR van 74,6% (95%-bti 61,8-85,0). De mediane PFS was 3,5 maanden
(95%-bti 2,4-3,8) en de mediane OS was 7,9 maanden (95%-bti 5,1-10,2). De PFS
was significant langer in de groep patiënten met graad 2 of hoger cutane
toxiciteit dan in de groep patiënten met graad 1 of geen cutane toxiciteit (HR
0,55; p=0,020). De meest-waargenomen ernstige toxiciteiten waren rash (37%),
tromboëmbolie (35,5%), en neutropenie (32%).
De
onderzoekers concluderen dat de triplet-combinatie gemcitabine, trastuzumab, en
erlotinib actief was als eerstelijns behandeling voor metastatisch pancreasadenocarcinoom,
met acceptabele veiligheid.
