Patiënten
met recidiverend of refractair B-cel ALL (R/R B-ALL) hebben behoefte aan
effectieve behandelingen die een brug kunnen vormen naar potentieel curatieve
hematopoïetische stamceltransplantatie of CAR T-celtherapie. In studies met
volwassen patiënten is werkzaamheid gezien van inotuzumab ozogamicine (InO). De
multicenter fase 2-studie AALL1621 van de Children’s
Oncology Group heeft InO geëvalueerd voor R/R CD22-positief B-ALL in
kinderen en adolescenten (leeftijd 1 tot en met 21 jaar). Dr. Maureen O’Brien
(University of Cincinnati OH) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 48 patiënten die evalueerbaar waren voor respons en toxiciteit. Na
één 28-daagse cyclus (intraveneus InO 0,8 mg/m2 op dag één en 0,5
mg/m2 op dagen acht en vijftien) hadden negentien patiënten hadden
complete respons (CR) en negen CR met incompleet herstel (percentage met
CR/CRi: 58,3%; 90%-bti 46,5-69,3). Onder deze 28 responders werd in 27 minimaal
residuele ziekte bepaald. In 18 van 27 patiënten (67%) was de MRD lager dan
0,01%. Drie patiënten hadden graad 3 ALT-verhoging en één had graad 3
hyperbilirubinemie. Onder 21 patiënten die na InO overgingen naar HSCT werd
graad 3 sinusoïdaal obstructiesyndroom (SOS) gezien in 6 patiënten (28,6%).
De
onderzoekers concluderen dat InO effectief was voor R/R CD22-positief B-ALL in
kinderen en adolescenten. SOS na HSCT was een significante toxiciteit.

Summary: Effective
and safe therapies that bridge to potentially curative HSCT or CAR T-cell
therapy are critical to improving outcomes for patients with relapsed or
refractory B-cell acute lymphoblastic leukemia. Although inotuzumab ozogamicin
(InO) is effective in adults, studies in children are limited. The multicenter phase II AALL1621 trial by the Children’s Oncology Group found that InO was well tolerated and
effective with high response rates and minimal residual disease <0.01% in
two thirds of responders. A significant proportion of responding patients
proceeded to HSCT and/or CAR T-cell therapy.
