Sotorasib is
een irreversibele remmer van het KRAS-eiwit met G12C-verandering. In de eerste
fase van de multinationale CodeBreaK100-studie is klinische activiteit gezien
van sotorasib monotherapie voor tumoren met KRAS-G12C
mutatie, inclusief colorectaalcarcinoom (CRC). Prof. Marwan Fakih (City of Hope
Comprehensive Cancer Center, Duarte CA) en collega’s publiceren in The Lancet Oncology een
vooraf-gespecificeerde analyse van de CRC-patiënten in fase 2 van de studie.1
CodeBreaK100
werd uitgevoerd in 59 centra in elf landen. Het CRC-cohort bestond uit
patiënten van 33 centra in negen landen. De patiënten waren achttien jaar of ouder,
hadden tenminste één meetbare lesie (RECIST 1.1), hadden een ECOG performance
status 0 of 1, en waren eerder behandeld met fluoropyrimidine, oxaliplatine, en
irinotecan. De patiënten kregen oraal sotorasib 960 mg eenmaal daags tot
progressie of niet-acceptabele toxiciteit. Het primaire eindpunt van de studie
was centraal-beoordeelde objectieve respons.
Op het
moment van data cutoff voor de nu gepubliceerde analyse waren 62 patiënten met KRAS-G12C gemuteerd CRC geïncludeerd. Objectieve
respons werd gezien in zes patiënten (9,7%; 95%-bti 3,6-19,9), allen met
partiële respons. Graad 3 treatment-related
adverse events werden gezien in zes patiënten (9,7%) en graad 4 TRAE in één
(2%), er waren geen graad 5 TRAEs. Ernstige TRAEs werden gezien in twee
patiënten (rugpijn en acute nierschade).
De
onderzoeker concluderen dat 9,7% ORR did
not reach the benchmark, maar dat sotorasib antitumor-activiteit had met
een manageable veiligheidsprofiel in deze zwaar-voorbehandelde patiënten.
Sotorasib wordt nader geëvalueerd als onderdeel van combinatiebehandelingen.

Summary: The
phase 1-2 CodeBreaK100 trial evaluated sotorasib monotherapy for KRAS G12C-mutated tumors. Prespecified
analysis of previously treated CRC-patients in phase 2 found an overall
response rate of 9.7%, which did not reach the benchmark. Even so, the authors
conclude that oral administration of sotorasib once per day showed modest
antitumor activity and manageable safety in these heavily pretreated chemorefractory
patients. Sotorasib is under evaluation in combination with other therapeutics
to increase potential activity and overcome potential resistance mechanisms.
