In de literatuur vonden de onderzoekers vijftien voor het onderwerp relevante gerandomiseerde gecontroleerde studies, met tezamen 1216 patiënten. Onder patiënten met EGFR-gemuteerd NSCLC met BMs was voor het eindpunt progressievrije overleving osimertinib de meest-werkzame behandeling, vergeleken met eerstegeneratie EGFR-TKI (HR 0,36; 95%-bti 0,38-0,55), tweedegeneratie EGFR-TKI (0,59; 0,34-0,99), conventionele chemotherapie (0,30; 0,14-0,66), radiotherapie (0,20; 0,14-0,29), en radiotherapie plus eerstegeneratie TKI (0,21; 0,14-0,32). Ook voor het eindpunt overall survival was osimertinib de meest-werkzame behandeling.
Onder patiënten met niet voor EGFR-mutatie geselecteerd NSCLC met BMs was voor het eindpunt progressievrije overleving de combinatie van anti-PD1 monoklonaal antilichaam plus conventionele chemotherapie de meest-werkzame behandeling, vergeleken met radiotherapie (HR 0,20; 95%-bti 0,14-0,29), conventionele chemotherapie (0,42; 0,28-0,68), radiotherapie plus conventionele chemotherapie (0,59; 0,32-0,98), radiotherapie plus eerstegeneratie TKI (0,49; 0,25-0,96), en immuuncheckpointremmer monotherapie (0,44; 0,28-0,69). Combinatietherapie was echter geassocieerd met verhoogd risico van toxiciteit.
De onderzoekers concluderen dat osimertinib de meest-werkzame behandeling is voor EGFR-gemuteerd NSCLC met BMs , terwijl de combinatie van anti-PD1 monoklonaal antilichaam plus conventionele chemotherapie de meeste activiteit had voor niet voor EGFR-mutatie geselecteerd NSCLC met BMs.

Summary: Network meta-analysis showed that osimertinib is the most effective and safe treatment for EGFR mutated NSCLC with brain metastases. For EGFR unselected NSCLC with BMs the combination of anti-PD1 monoclonal antibody plus conventional chemotherapy was most effective but also associated with increased toxicity.
