De Chinese
multicenter fase 3-studie ORIENT-11 randomiseerde patiënten met niet-eerder
behandeld lokaal-gevorderd of metastatisch niet-squameus niet-kleincellig
longcarcinoom (nsqNSCLC) 2:1 naar vier cycli sintilimab 200 mg (anti-PD-1) plus
pemetrexed-platina chemotherapie iedere drie weken gevolgd door sintilimab
onderhoudstherapie (n=266) of vier cycli placebo plus pemetrexed-platina
chemotherapie gevolgd door placebo onderhoudstherapie (n=131). Cross-over van
placebo naar sintilimab na progressie was toegestaan. In 2020 is gepubliceerd dat met mediaan 8,9 maanden follow-up de progressievrije
overleving (primair eindpunt) significant beter was in de sintilimabgroep dan
in de placebogroep (mediaan 8,9 versus 5,0 maanden; HR 0,482; p<0,00001).

resultaten en predictieve biomarkeranalyses van de studie.1
De analyses
werden uitgevoerd na mediaan 22,9 maanden follow-up. De mediane overall
survival was niet bereikt in de sintilimabgroep versus 16,8 maanden in de
placebogroep (HR 0,60; p=0,0003). Hoge of intermediaire immuuncelinfiltratie in
de tumoren, vergeleken met lage of geen infiltratie, was sterk geassocieerd met
betere PFS in de sintilimabgroep. Hoge expressie van major histocompatibility complex MHC)-II presentatieroute was
significant geassocieerd met langere PFS (HR 0,32; p<0,0001) en OS (HR 0,36;
p=0,0005) in de sintilimabgroep. Patiënten met lage of geen expressie van PD-L1
maar hoge expressie van MHC-II hadden profijt van sintilimab-behandeling.
De
onderzoekers concluderen dat toevoeging van sintilimab aan eerstelijns
chemotherapie resulteerde in significante verlenging van de OS onder patiënten
met lokaal-gevorderd of metastatisch nsqNSCLC.

Summary: The
Chinese multicenter phase 3 ORIENT-11 study randomized patients with previously untreated locally advanced
non-squamous NSCLC to sintilimab (anti-PD-1) plus chemotherapy or placebo plus
chemotherapy; with cross-over allowed from placebo to sintilimab at progression.
After median 22.9 months of follow-up the median overall survival was not
reached in the sintilimab group versus 16.8 months the placebo group (HR 0.60;
p=0.0003). Expression of MHC class-II antigen presentation pathway identified
patients benefitting most from addition of sintilimab to chemotherapy.
