Thiotepa-busulfan-fludarabine
(TBF) is een veel-gebruikt conditioneringsregime in single umbilical cord blood transplantation (SUCBT). Recentelijk is
TBF ook geïntroduceerd in de setting van non-T cell depleted haploïdentieke
stamceltransplantatie (NTD-Haplo). Een multicenter studie van de European Group for Bone and Marrow
Transplantation heeft uitkomsten
vergeleken van NTD-Haplo (n=186) versus SUCBT (n=147) voor patiënten met de novo AML in complete remissie na
myeloablatieve conditionering met TBF. Prof. Arnon Nagler (Sheba Medisch
Centrum, Israël) en collega’s publiceren de studie online in het Journal of
Hematology & Oncology.1
De mediane
follow-up na transplantatie was 23 maanden. SUCBT was vergeleken met NTD-Haplo
geassocieerd met latere engraftment en meer graft
failure. In multivariate analyse waren er tussen beide groepen geen
statistisch significante verschillen in acute GVHD (HR 1,03; p=0,92),
chronische GVHD (HR 1,86; p=0,21), en incidentie van relapse (HR 0,8; p=0,65).
De non-relapse mortaliteit was significant hoger in de SUCBT-groep dan in de
NTD-haplo groep (HR 2,63; p=0,001). De SUCBT-groep had ook slechtere overall survival (HR 2,18; p=0,002),
leukemievrije overleving (HR 1,94; p=0,007), en GVHD relapse-vrije overleving
(HR 2,38; p=0,0002).
De
onderzoekers concluderen dat myelo-ablatieve conditionering van TBF sterke
graft-versus-leukemie werkzaamheid mogelijk maakt, ongeacht het donortype. In
patiënten die TBF conditionering kregen waren de overleving en de non-relapse
mortaliteit met NTD-Haplo beter dan met SUCBT.

Summary: A multicenter study from the European
Group for Blood and Marrow Transplantation showed that after conditioning with
thiotepa-busulfan-fludarabine stem cell transplantation allows for potent
graft-versus-leukemia efficacy, regardless of the donor type (NTD-Haplo or
SUCBT). Survival with NTD-Haplo may be better compared to SUCBT due to
decreased non-relapse mortality.
