Een gepoolde
analyse van vier studies onderzocht de prognostische waarde van diepte van
respons van BRAFV600-gemuteerd metastatisch melanoom op behandeling
met vemurafenib met of zonder cobimetinib (hoewel de titel van de publicatie
ten onrechte behandeling met cobimetinib met of zonder vemurafenib meldt). Prof.
Karl Lewis (University of Colorado, Aurora) en collega’s publiceren de analyse online in het British Journal of
Cancer.1 De betreffende studies zijn BRIM-2, BRIM-3, BRIM-7, en
coBRIM.
De analyse
laat zien dat grotere tumorreductie en langere tijd tot maximale respons
geëvalueerd als continue variabelen significant geassocieerd waren met langere
progressievrije overleving en overall
survival. Patiënten met de diepste responsen hadden langdurige
overlevingsuitkomsten (vemurafenib monotherapie: mediane PFS 14 maanden en
mediane OS 32 maanden; vemurafenib plus cometinib: mediane PFS en OS
niet-bereikt). Cobimetinib plus vemurafenib vergeleken met vemurafenib
monotherapie was geassocieerd met diepere respons ongeacht andere prognostische
factoren, waaronder gensignaturen.
De
onderzoekers concluderen dat grotere diepte van respons geassocieerd was met
betere overleving, hetgeen gebruik van diepte van respons als maat voor werkzaamheid
van behandeling steunt. De combinatie van cobimetinib plus vemurafenib
verbeterde uitkomsten over kwartielen van respons ongeacht andere prognostische
factoren.
