Sorafenib is
een potente multikinaseremmer met werkzaamheid voor AML als monotherapie. Een
studie van MD Anderson Cancer Center (Houston TX) heeft de overlevingsimpact
onderzocht van toevoegen van sorafenib aan intensieve inductiechemotherapie
voor nieuw-gediagnostiseerd FTL3-internal
tandem duplication (ITD)-gemuteerd AML. Prof. Farhad Ravandi en collega’s
publiceren de studie online in Cancer.1
Tussen
februari 2001 en december 2017 werden in het ziekenhuis van MD Anderson 183
patiënten met nieuw-gediagnostiseerd FLT3-ITD
gemuteerd AML behandeld: 79 patiënten kregen sorafenib plus intensieve
chemotherapie, en 104 alleen intensieve chemotherapie. Propensity score
matching resulteerde in 42 patiënten in elk van beide cohorten. De ORR was 98%
in het sorafenib plus intensieve chemotherapie cohort versus 83% in het
alleen-intensieve chemotherapie cohort (p=0,057). De mediane follow-up was 54
maanden. De mediane gebeurtenisvrije overleving was 35 maanden in het sorafenib
plus intensieve chemotherapie cohort versus 8 maanden in het alleen intensieve
chemotherapie cohort (p=0,019) en de mediane overall survival was 42 versus 13 maanden. Met censoring op het
moment van allogene stamceltransplantatie was de mediane EFS 31 versus 8
maanden (p=0,031) en de mediane OS niet-bereikt versus 10 maanden (p=0,001). In
multivariate analyse was toevoegen van sorafenib aan intensieve chemotherapie
geassocieerd met significante verbetering van de OS (HR 0,558; p=0,009).
De onderzoekers
concluderen dat toevoegen van sorafenib aan intensieve chemotherapie voor FLT3-ITD gemuteerd AML resulteerde in
verbetering van de uitkomsten ongeacht of de patiënten allogene
stamceltransplantatie ondergaan.

event-free survival and overall survival regardless of whether patients undergo
allogeneic stem cell transplantation.
