Er is geen
duidelijkheid over de optimale post-remissie therapie voor Ph-negatief ALL in
adolescenten en jongvolwassenen (AYAs; leeftijd zestien tot veertig jaar). Een
analyse van patiënten in de CALGB 10403-studie heeft de overlevingsimpact
vergeleken van pediatrische-stijl chemotherapie versus myeloablatieve allogene
hematopoïetische stamceltransplantatie (MA alloHCT) voor Ph-negatief ALL in
AYAs in eerste complete remissie (CR1). Dr. Matthew Wieduwilt (University of

Het
chemotherapie-cohort bestond uit 263 patiënten die werden behandeld in 60
centra. De mediane leeftijd was 24 jaar (range 17-40) en de mediane follow-up
was 65 maanden (range 1-109). Het MA alloHCT-cohort bestond uit 217 patiënten
die werden behandeld in 72 centra. De mediane leeftijd was 27 jaar (range 16-41)
en de mediane follow-up was 98 maanden (range 23-179). De figuur laat zien dat de MA alloHCT groep vergeleken met de
chemotherapiegroep inferieure uitkomsten had voor overall survival (HR 2,00; p<0,001), ziektevrije overleving (HR
1,62; p<0,001), en non-relapse mortaliteit (HR 5,41; p<0,001). De
vijf-jaars incidentie van relapse was hoger in de chemotherapiegroep dan in de
MA alloHCT groep (34% versus 23%; p=0,011). In beide groepen was obesitas
onafhankelijk geassocieerd met slechtere OS, DFS, en NRM.
De
onderzoekers concluderen dat onder AYAs met Ph-negatief ALL in eerste complete
remissie pediatrische-stijl chemotherapie vergeleken met MA alloHCT geassocieerd
was met betere OS, DFS, en NRM.

Chemotherapy was superior to MA alloHCT for overall survival, disease-free survival, and
non-relapse mortality. The incidence of relapse at five years was higher with
chemotherapy than with MA alloHCT.
