De fase
3-studie GOG-0218 liet zien dat toevoegen van bevacizumab aan frontline chemotherapie voor stadium III
of IV ovariumcarcinoom resulteerde in significante verbetering van de progressievrije
overleving maar niet van de overall
survival. Prof. Michael Birrer (Massachusetts General Hospital, Boston) en
collega’s hebben een exploratieve analyse uitgevoerd van kandidaat-biomarkers
voor bevacizumab-profijt in GOG-0218. Ze publiceren de analyse online in het Journal of the
National Cancer Institute.1
GOG-0218
randomiseerde patiënten met stadium III of IV ovariumcarcinoom naar zes cycli
chemotherapie met placebo of bevacizumab, gevolgd door single-agent placebo of bevacizumab. Biomarkers konden geëvalueerd
worden in 980 deelneemsters (78,5% van de ITT-populatie) met voor de gehele populatie
representatieve baseline-kenmerken en uitkomsten. De analyse liet geen
prognostische of predictieve associaties zien voor VEGFR-2, neurofiline-1, of
MET. Hogere microvessel-dichtheid (MVD, bepaald met CD31) had predictieve
waarde voor PFS en OS. De PFS-HR voor bevacizumab versus placebo was 0,40 voor
hoge MVD versus 0,80 voor lage MVD (p voor interactie 0,003). De OS-HR voor
bevacizumab versus placebo was 0,67 voor hoge MVD versus 1,10 voor lage MVD (p
voor interactie 0,02). Tumor VEGF-A was niet predictief voor PFS maar mogelijk
wel predictief voor OS, bij gebruik van derde-kwartiel afsnijwaarde voor hoge
VEGF-A expressie.
De
onderzoekers concluderen dat deze retrospectieve tumorbiomarker-analyses een
positieve associatie suggereren tussen dichtheid van vasculaire endotheelcellen
en tumor VEGF-A niveaus en werkzaamheid van bevacizumab in ovariumcarcinoom.
