Patiënten
met HER2-positief mammacarcinoom kunnen hersenmetastasen (BCBM) ontwikkelen .
Prognostische stratificatie van patiënten met HER2-positief BCBM is van belang
voor optimaal management. Een studie van het Istituto Veneto-Padova (Italië) en
het Institut du Cancer de Montpellier (Frankrijk) heeft de prognostische impact
van extracraniële ziektecontrole in deze patiënten geïnventariseerd. Dr. Gaia
Griguolo (IVP) en collega’s publiceren de studie in het British Journal of Cancer.1
De studie
includeerde 113 Italiaanse patiënten met HER2-positief BCBM in een exploratiecohort
en 117 Franse patiënten in een validatiecohort. De figuur laat zien dat in beide cohorten extracraniële controle geassocieerd
was met betere overall survival: In
Italië was de mediane OS 17,7 maanden in de groep met extracraniële controle
versus 8,7 maanden in de groep zonder extracraniële controle (p=0,005); in
Frankrijk was de mediane OS 20,2 versus 9,1 maanden (p<0,001). In
multivariate analyse na correctie voor breast
cancer specific grade prognostic assesment (BC-GPA) score bleef
extracraniële controle prognostisch in het exploratiecohort (HR 0,61; 95%-bti
0,39-0,94) en het validatiecohort (0,41; 0,27-0,61).
De
onderzoekers concluderen dat onder patiënten met BCBM extracraniële
ziektecontrole onafhankelijke prognostische informatie levert.
