Een
bevolkings-gebaseerde retrospectieve cohortstudie in Ontario (Canada) heeft het
risico van hospitalisatie voor hartfalen en andere cardiovasculaire ziekten
onderzocht in vrouwen na een diagnose vroeg-stadium mammacarcinoom (EBC). Dr.
Husam Abdel-Qadir (Women’s College Hospital, Toronto) en collega’s publiceren
de studie online in het Journal of the
National Cancer Institute.1 De studie includeerde 78.317 vrouwen
met een diagnose EBC tussen 1 april 2005 en 31 maart 2015, 1:3 voor
geboortejaar gematcht met vrouwen zonder maligniteit (n=234.954).
De
tien-jaars incidentie van CVD-hospitalisatie was 10,8% (95%-bti 10,5-11,1) in
de EBC-groep versus 9,1% (95%-bti 8,9-9,2) in de controlegroep. Ischemische
hartziekte was de meest-frequente reden voor CVD-hospitalisatie na EBC. Na
regressiecorrectie bleven in het EBC-cohort de relatieve risico’s statistisch
significant verhoogd van hospitalisatie voor hartfalen (HR 1,21; p<0,001),
aritmie (HR 1,31; p<0,001), en cerebrovasculaire ziekte (HR 1,10; p=0,002).
Slechts in 2,9% van de EBC-patiënten die werden gehospitaliseerd voor hartfalen
bestonden geen bekende risicofactoren (leeftijd hoger dan zestig jaar,
hypertensie, diabetes, eerdere CVD). Anthracyclines en/of trastuzumab werden
gebruikt in 28.950 EBC-patiënten; vergeleken met het gehele cohort waren deze
patiënten jonger, met lagere absolute incidentie van CVD, hypertensie, en
diabetes. Vergeleken met voor leeftijd gematchte controlepersonen hadden ze
echter een verhoogd risico van CVD.
De
onderzoekers concluderen dat ahterosclerotische diagnoses, meer dan hartfalen,
de meest-frequente renen voor CVD-hospitalisatie na EBC waren. Hospitalisatie voor
hartfalen werd in de meeste gevallen voorafgegaan door risicofactoren anders
dan chemotherapie, hetgeen mogelijkheden voor preventie suggereert.
