Branch-duct intraductal papillary mucinous
neoplasms (BD-IPMNs) zijn veel-voorkomende preneoplastische lesies in de
pancreas. Laag-risico BD-IPMNs kunnen uitgroeien tot BD-IPMNs met zorgwekkende
kenmerken (worrisome features, WF) of
high-risk stigmata (HRS) BD-IPMNs.
Een cohortstudie in Italië heeft risicofactoren voor progressie tot het WF- of
HRS-stadium geïnventariseerd. Prof. Paolo Giorgio Arcidiacono (Wetenschappelijk
Instituut San Raffaele, Milaan) en collega’s publiceren de studie in JAMA Network Open.1
De studie
includeerde 540 patiënten met laag-risico BD-IPMNs, die tenminste een jaar
gevolgd werden voor progressie. De mediane leeftijd was 66 jaar (IQR
58,5-72,0); 62,4% van de patiënten waren vrouwen, en de mediane surveillance
duurde 51,5 maanden (IQR 28-84). Tijdens 2758 patiëntjaren follow-up werd
progressie gezien in 3,7% na één jaar, 23,4% na vijf jaar, en 43,3% na tien
jaar. Risicofactoren voor progressie waren initiële cystegrootte meer dan 15 mm (HR
2,05; 95%-bti 1,44-2,91), BMI hoger dan 26,4 kg/m2 (HR 1,72; 95%-bti
1,19-2,50), en heavy smoking (HR
1,81; 95%-bti 1,14-2,86). In een subgroep van patiënten met bekend
bloed-genotype was het AA-genotype geassocieerd met verhoogd risico van
progressie (versus OO: HR 3,49; 95%-bti 1,04-11,71).
De
onderzoekers concluderen dat cystegrootte alleen geen betrouwbare factor is
voor het schatten van het risico van progressie.

Summary: A cohort study investigated
risk factors for progression of low-risk branch-duct intraductal papillary
mucinous neoplasms into worrisome features or high-risk stigmata categories of
BD-IPMNs. Probability of progression was 3.7% at 1 year follow-up, 23.4% at 5
years, and 43.3% at 10 years. Risk factor for progression were
initial cyst size greater than 15 mm, BMI greater than 26.4, and heavy smoking.
The authors conclude that initial cyst size alone is not a reliable factor for
estimation of progression risk.
