De gerandomiseerde
fase 3-studie IELSG-19 onderzocht de waarde van eerstelijns behandeling met
rituximab plus chlorambucil versus rituximab monotherapie versus chlorambucil
monotherapie voor extranodal marginal
zone lymphoma of mucosa-associated lymphoid tissue (MALT lymfoom). Dr.
Emanuele Zucca (Universiteit van Zuid-Zwitserland, Bellinzona) en collega’s publiceren
de studie online in het Journal of Clinical
Oncology.1 De studie randomiseerde de patiënten aanvankelijk
naar chloramubcil monotherapie of chlorambucil plus rituximab. Nadat 252
patiënten waren gerandomiseerd werd het protocol gewijzigd en werd een derde
arm (rituximab monotherapie) aan de studie toegevoegd. De onderzoekers
publiceren nu de uitkomsten van 132 patiënten in de combinatie-arm, 131
patiënten in de chlorambucil-monotherapie arm, en 138 patiënten in de
rituximab-monotherapie arm.
De mediane follow-up was 7,4 jaar. De figuur laat zien dat in de combinatie-arm vergeleken met beide
monotherapie-armen de gebeurtenisvrije overleving en progressievrije overleving
significant beter waren. Er was echter geen statistisch significant verschil in
overall survival tussen de armen. De
vijf-jaars OS was ongeveer 90% in iedere arm. De behandeling werden goed
verdragen, zonder onverwachte toxiciteiten.
De
onderzoekers concluderen dat rituximab plus chlorambucil vergeleken met beide
middelen als monotherapie superieure werkzaamheid heeft voor MALT-lymfoom, maar
dat de betere EFS en PFS in de combinatie-arm niet resulteerde in betere OS.
