Er is geen
duidelijkheid over de optimale techniek voor accelerated partial breast irradiation (APBI) voor patiënten die
borstsparende chirurgie (BCS) hebben ondergaan voor vroeg-stadium
mammacarcinoom (EBC). Een fase 3-noninferioriteitsstudie in zestien centra in
zeven Europese landen heeft APBI met multicatheter brachytherapie vergeleken
met gehele-borstbestraling (WBI) met boost. Prof. Vratislav Strnad
(Universiteitsziekenhuis Erlangen, Duitsland) en collega’s publiceren in The Lancet Oncology tien-jaars
follow-up resultaten van de studie.1
De studie
includeerde 1328 patiënten na BCS voor EBC. De patiënten werden 1:1
gerandomiseerd naar WBI plus boost (n=673) of APBI (n=655). Het primaire
eindpunt was ipsilateraal recidief. De mediane follow-up op het moment van de
nu gepubliceerde analyse was 10,36 jaar (IQR 9,12-11,28). De tien-jaars percentages
patiënten met lokaal-recidief waren 1,58% in de WBI-groep versus 3,51% in de
APBI-groep; een verschil van 1,93% (95%-bti -0,018 tot 3,87; p=0,074). Adverse events waren voornamelijk graad
1 of 2, in 60% van de WBI-groep en 67% van de APBI-groep. De APBI-groep had een
significant lagere incidentie van behandelings-gerelateerde graad 3 late
bijwerkingen dan de WBI-groep (1% versus 4%; p=0,021); in beide groepen
voornamelijk fibrose. Er waren geen graad 4 of 5 behandelings-gerelateerde
bijwerkingen.
De
onderzoekers concluderen dat APBI met multicatheter brachytherapie na BCS voor
EBC een waardevol alternatief voor WBI is in termen van werkzaamheid en
geassocieerd is met minder late graad 3-bijwerkingen.

Summary: A randomized phase 3 trial at 16 centers in 7 European countries compared
accelerated partial breast irradiation (APBI) using sole interstitial
multicatheter brachytherapy versus whole-breast irradiation (WBI) with boost after
breast-conserving surgery for early-stage breast cancer. At ten years, there
was no significant difference between the two groups in terms of ipsilateral
local recurrence (p=0.074) or grade 1 or 2 adverse events. The incidence of late
grade 3 treatment-related adverse events was lower in the APBI group than in
the WBI group (1% versus 4%; p=0.021).
