Het is niet duidelijk of resectie van de primaire tumor (PTR) voorafgaand aan chemotherapie resulteert in overlevingsvoordeel onder patiënten met colorectaalcarcinoom (CRC) met asymptomatische primaire tumor en synchrone niet-resectabele metastasen. De Japanse multicenter fase 3-studie JCOG1007 heeft PTR gevolgd door chemotherapie in deze patiënten vergeleken met alleen chemotherapie. Dr. Yikehide Kanemitsu (Ziekenhuis van het Nationaal Kankercentrum, Tokio) en collega’s publiceren de eerste interimanalye van de studie in het Journal of Clinical Oncology.¹

De studie includeerde tussen juni 2012 en september 2019 patiënten met stadium IV asymptomatisch CRC met ten hoogste drie niet-resectabele metastasen in lever, longen, afstands-lymfeklieren, of peritoneum. De patiënten werden gerandomiseerd naar PTR plus chemotherapie (mFOLFOX6 plus bevacizumab of CapeOx plus bevacizumab; n=81) of alleen chemotherapie (n=84). De eerste interimanalyse werd uitgevoerd in september 2019. De figuur laat zien dat er geen verschillen waren tussen beide armen in overall survival of progressievrije overleving. Drie patiënten in de PTR-groep overleden aan oorzaken die met de chirurgie samenhingen. De studie is voortijdig beëindigd wegens futiliteit.

De onderzoekers concluderen dat PTR voorafgaand aan chemotherapie niet resulteerde in verbetering van de uitkomsten van CRC-patiënten met asynptomatische primaire tumor en synchrone niet-resectabele metastasen.
