De multinationale
fase 3-studie SOLO1 includeerde vrouwen met nieuw-gediagnostiseerd gevorderd (FIGO-stadium
III of IV) ovariumcarcinoom en een BRCA1/2
–mutatie met complete of partiële respons na chirurgie en platina-gebaseerde
chemotherapie. De patiënten werden 2:1 gerandomiseerd naar olaparib-onderhoud
(300 mg tweemaal daags) of placebo. In 2018 is gepubliceerd dat de progressievrije overleving in de olaparibgroep significant
beter was dan in de placebogroep (na drie jaar 60% versus 27%; HR 0,30;
p<0,001). Dr. Paul DiSilvestro (Women & Infants Hospital, Providence RI)
en collega’s publiceren nu in het Journal of Clinical
Oncology analyses van resultaten in SOLO1-subgroepen.1
Olaparib
vergeleken met placebo verlaagde het risico van ziekteprogressie in zowel de
groep patiënten met upfront chirurgie (HR 0,31; 95%-bti 0,21-0,46) als met
interval-chirurgie (0,37; 0,24-0,58), zowel in patiënten met residuele ziekte
na chirurgie (0,44; 0,25-0,58) als zonder residuele ziekte na chirurgie (0,33;
0,23-0,46), zowel in patiënten met baseline complete respons (0,34; 0,24-0,47)
als in patiënten met baseline partiële respons (0,31; 0,18-0,52), en zowel in
patiënten met BRCA1-mutatie (0,41;
0,30-0,56) als in patiënten met BRCA2-mutatie
(0,20;0,10-0,37).
De
onderzoekers concluderen dat vrouwen met gevorderd ovariumcarcinoom en een BRCA-mutatie substantieel profijt hadden
van olaparib-onderhoud ongeacht resultaat van chirurgie, respons op
chemotherapie, en type BRCA-mutatie.

Summary: Subgroup analyses of the
multinational phase 3 study SOLO1 showed that, after surgery and chemotherapy
for newly diagnosed advanced ovarian cancer, patients with BRCA1/2 mutations achieved substantial benefit from maintenance
olaparib regardless of baseline surgery outcome, response to chemotherapy, or
BRCA mutation type.
