De waarde
van profylactische whole-pelvic nodal radiation
therapy (WPRT) voor hoog-risico prostaatcarcinoom is niet duidelijk. De fase
3-studie POP-RT van Tata Memorial Hospital (Mumbai, India) heeft uitkomsten van
WPRT vergeleken met uitkomsten van prostate-only
radiotherapy (PORT) voor kliernegatief prostaatadenocarcinoom met geschat
nodaal risico 20% of hoger. Prof. Vedang Murthy en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 224 patiënten die IMRT en tenminste twee jaar androgeendeprivatietherapie
kregen. De patiënten werden 1:1 gerandomiseerd naar PORT (68 Gy in 25 fracties
naar de prostaat; n=114) of WPRT (68 Gy in 25 fracties naar de prostaat plus 50
Gy in 25 fracties naar de pelvische klieren; n=110). Randomisatie geschiedde
gestratificeerd naar Gleason score, type ADT, PSA-waarde bij diagnose, en
eerdere transurethrale resectie. Het primaire eindpunt was percentage patiënten
zonder biochemisch falen na vijf jaar (BFFS). Secundaire eindpunten waren vijf-jaars
ziektevrije overleving en overall
survival.
De mediane
follow-up was 68 maanden. De vijf-jaars BFFS was 95,0% in de WPRT-groep versus
81,2% in de PORT-groep (HR 0,23; p<0,001). De vijf-jaars DFS was 89,5% met
WPRT versus 77,2% met PORT (HR 0,40; p=0,002), evenals de vijf-jaars afstandsmetastasevrije
overleving (95,9% met WPRT versus 89,2% met PORT; HR 0,35; p=0,01), maar er was
geen statistisch significant verschil in vijf-jaars OS: 92,5% met WPRT versus
90,8% met PORT (HR 0,92; p=0,83). Het profijt van WPRT vergeleken met PORT voor
BFFS en DFS werd gezien in de prognostische subgroepen.
De
onderzoekers concluderen dat profylactische WPRT vergeleken met PORT voor
hoog-risico prostaatadenocarcinoom resulteerde in betere BFFS en DFS maar niet
OS.
