De studie includeerde volwassen patiënten met gevorderde MSI-H/dMMR tumoren. Patiënten die niet eerder anti-PD-(L)1 therapie hadden gekregen werden behandeld met lodapolimab monotherapie (n=40) of lodapolimab plus LY3321367 (n=20), en patiënten met anti-PD-(L)1 refractaire tumoren kregen de combinatie (n=22; meer dan 70% na drie of meer eerdere behandelingen). Lodapolimab 700 mg en LY3321367 (eerste twee cycli 1200 mg, vervolgens 600 mg) werden iedere twee weken intraveneus gegeven. Primaire eindpunten waren veiligheid en tolerabiliteit; een secundair eindpunt was percentage patiënten met objectieve respons.
Van de 82 geïncludeerde patiënten hadden de meesten colorectale (47,6%) of endometrium (17,1%) tumoren. Treatment-related adverse events werden gezien in 22 patiënten (55,0%) die monotherapie kregen, 13 patiënten (65,0%) in het PD-(L)1 remmer-naïeve combinatiecohort, en 6 patiënten (27,3%) in het PD-(L)1-remmer resistente/refractaire cohort. Graad 3 of hoger TRAEs werden gezien in 2 patiënten (5,0%) die monotherapie kregen en 3 patiënten (7,1%) die combinatietherapie kregen. Objectieve responsen werden gezien in 13 patiënten (32,5%) met monotherapie, 9 patiënten (45%) in het PD-(L)1 remmer-naïeve combinatiecohort, en één patiënt (4,5%) in het PD-(L)1 remmer resistente/refractaire cohort.
De onderzoekers concluderen dat lodapolimab monotherapie en in combinatie met LY3321367 een manageable veiligheidsprofiel had. Beide regimes hadden veelbelovende klinische activiteit voor PD-(L)1 remmer-naïeve gevorderde solide MSI-H/dMMR tumoren; de combinatie had slechts beperkte activiteit voor PD-(L)1 remmer-resistente/refractaire MSI-H/dMMR tumoren.

Summary: A multinational phase 1b study found that the anti-PD-L1 antibody lodapolimab as monotherapy and in combination with the anti-TIM-3 antibody LY3321367 had manageable safety profiles and had promising clinical activity for PD-(L)1 inhibitor-naïve MSI-H/dMMR advanced solid tumors. The combination had limited activity for PD-(L)1 inhibitor-resistant/refractory disease.
