Chemotherapie-geïnduceerde
beschadiging van hematopoïetische stam- en progenitorcellen (HSPC) leidt tot
myelosuppressie. Trilaciclib is een intraveneus toegediende CDK4/6-remmer, die
in preklinische studies HSPC transiënt in G1-arrest hield en beschermde tegen
chemotherapie-geïnduceerde beschadiging. De multinationale fase 1b/2-studie
GIT28-02 heeft de waarde onderzocht van toevoeging van trilaciclib aan
eerstelijns chemotherapie voor kleincellig longcarcinoom. Prof. Konstantin
Dragnev (Dartmouth-Hitchcock Medical Center, Lebanon NH) en collega’s
publiceren de studie online in Annals of Oncology.1
Fase 1b
includeerde 19 patiënten die verschillende doseringen trilaciclib kregen voorafgaand
aan chemotherapie (carboplatine-etoposide) op dagen één tot en met drie van
iedere cyclus. Als aanbevolen fase 2-dosering kozen de onderzoekers trilaciclib
240 mg/m2. In fase 2 werden 77 patiënten gerandomiseerd naar chemotherapie
plus trilaciclib (n=39) of chemotherapie plus placebo (n=38). Graad 3 of hoger adverse events werden gezien in 50% van
de patiënten in de trilaciclib-arm versus 83,8% van de patiënten in de
placebo-arm; dit verschil was vooral te verklaren uit minder hematologische
toxiciteit in de trilaciclib-arm. De antitumor-activiteit van de chemotherapie
werd niet gecompromitteerd door toevoeging van trilaciclib (ORR 66,6% met
trilaciclib versus 56,8% met placebo; mediane PFS 6,2 versus 5,0 maanden;
mediane OS 10,9 versus 10,6 maanden).
De
onderzoekers concluderen dat toevoeging van trilaciclib aan eerstelijns
chemotherapie voor SCLC resulteerde in beter verdragen van de chemotherapie
zonder dat de antitumor-werkzaamheid van de chemotherapie gecompromitteerd
werd.

Summary: The
multinational GIT28-02 study evaluated myelopreservation with the CDK4/6 inibitor trilaciclib added to
first-line chemotherapy for SCLC. Patients in the trilaciclib arm had better
tolerability of chemotherapy compared to patients in the placebo arm, while the
anti-tumor efficacy of the chemotherapy was not compromised by addition of trilaciclib.
