FoRT (Follicular Radiotherapy Trial) werd uitgevoerd in 43 centra. De studie includeerde 548 volwassen patiënten (614 radiotherapie target sites) met histologisch bevestigd FL of MZL. De patiënten werden gerandomiseerd naar standaard RT-dosering (n=299) of lage RT-dosering (n=315). Het primaire eindpunt was tijd tot lokale progressie in het bestraalde volume, met als noninferioriteits-criterium ten hoogste 10% slechtere uitkomst in de 4 Gy-arm na twee jaar.
De mediane follow-up was 73,8 maanden (IQR 61,9-88,0). Er werden in het cohort 117 lokale-progressiegebeurtenissen gezien; 27 in de 94-Gy groep en 90 in de 4-Gy groep. Het twee-jaars lokale-progressievrije percentage was 94,1% in de 24-Gy groep versus 79,8% in de 4-Gy groep, waarmee de 4-Gy behandeling niet voldeed aan het noninferioriteits-criterium. Na vijf jaar was het lokale-progressievrije percentage 89,9% versus 70,4% (HR 3,46; p<0,0001). De meest-gerapporteerde events na twaalf weken waren alopecie (7% van de patiënten met standaard-dosering versus 2% met lage-dosering), droge mond (4% versus 2%), vermoeidheid (2% versus 2%), mucositis (2% versus 1%), en pijn (2% versus 1%). Geen van de patiënten overleed aan een behandelingsgerelateerde oorzaak.
De onderzoekers concluderen dat 24 Gy in twaalf fracties de optimale radiotherapiedosering is voor indolent lymfoom (als duurzame lokale controle het doel van de behandeling is).

Summary: The multicenter phase 3 FoRT trial in the UK compared 4 Gy in two fractions versus 24 Gy in twelve fractions for follicular and marginal zone lymphoma. The 5-year local progression-free rate was 89.9% after 24 Gy versus 70.4% after 4 Gy (HR 3.46; p<0.0001). The authors conclude that the optimal radiotherapy dose for indolent lymphoma is 24 Gy in twelve fractions.
