Er is geen
duidelijkheid over het risico van borst-induratie na hypogefractioneerde
radiotherapie na borstsparende chirurgie (BCS) voor vroeg-stadium
mammacarcinoom (EBC) of DCIS. De Deense fase 3-studie DBCG HYPO heeft het
risico van induratie geïnventariseerd na standaard-behandeling versus
hypogefractioneerde behandeling. Prof. Birgitte Offersen (Universiteit van
Aarhus) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
DBCG HYPO
werd uitgevoerd in acht centra in Denemarken. Tussen begin 2009 en eind 2014
includeerde de studie 1854 patiënten, ouder dan 40 jaar, die BCS hadden
ondergaan voor kliernegatief EBC (n=1608) of DCIS (n-246). De patiënten werden
gerandomiseerd naar 50 Gy in 25 fracties (n=937) of 40 Gy in 15 fracties
(n=917). Het primaire eindpunt van de studie was graad 2 of 3 induratie na drie
jaar, onder veronderstelling van noninferioriteit van hypogefractioneerde
behandeling voor het eindpunt locoregionaal recidief.
Het
percentage patiënten met induratie na drie jaar was 11,8% in de 50-Gy groep
versus 9,0% in de 40-Gy groep (verschil -2,7%; 95%-bti -5,6 tot +0,2; p=0,07).
Systemische behandeling en RT-boost resulteerden niet in hoger risico van
induratie. Telangiëctasie, dyspigmentatie, scar
appearance, oedeem, en pijn waren infrequent, en er waren geen belangrijke
verschillen voor kosmetische resultaten en patiënttevredenheid. Het negen-jaars
van locoregionaal recidief was 3,3% in de 50-Gy groep versus 3,0% in de 40-Gy
groep (verschil -0,3%; 95%-bti -2,3 tot +1,7), en de negen-jaars overall survival was 93,4% in beide
groepen. Voorkomen van stralings-gerelateerde hart- en longziekte was zeldzaam,
en niet verschillend tussen beide groepen.
De
onderzoekers concluderen dat matig-hypogefractioneerde radiotherapie voor
kliernegatief EBC of DCIS niet resulteerde in meer borst-induratie dan
standaard-gefractioneerde therapie. Andere effecten op normale weefsels waren
infrequent, en niet slechter in de hypogefractioneerde groep. Het negen-jaars
risico van locoregionaal recidief was laag.

cancer or DCIS moderately hypofractionated RT (40 Gy in 15 fractions) did not
increase the risk of breast induration compared with standard fractionated RT
(50 Gy in 25 fractions). The 9-year locoregional recurrence risk was low (3.3%
in the 50-Gy group versus 3.0% in the 40-Gy group; NS) and the 9-year overall
survival was 93.4% in both groups..
