Er is geen
duidelijkheid over de waarde van consolidatie-radiotherapie (cRT) na complete
metabole respons op ABVD-chemotherapie voor gevorderd Hodgkin lymfoom (HL) met large nodal mass (LNM; gedefinieerd als
langste diameter tenminste 5 cm). De Italiaanse multicenterstudie HD 0607 heeft
deze waarde onderzocht. Prof. Andrea Gallamini (Centrum A. Lacassagne, Nice,
Frankrijk) en collega’s publiceren de studie in het Journal of Clinical
Oncology.1
De studie
includeerde 296 patiënten met gevorderd (stadium IIB-IVB) HL met LNM voor de
start van ABVD. De langste diameter was 5 tot 8 cm in 101 patiënten (34%;
subgroep A), 8 tot 10 cm in 96 patiënten (32%; subgroep B), en langer dan 10 cm
in 99 patiënten (33%; subgroep C). Er waren 280 patiënten (88%) met negatieve
PET na twee en na zes cycli ABVD. Deze patiënten werden prospectief
gerandomiseerd naar cRT naar de LNM (mediaan 30,6 Gy; range 24-36) of geen
verdere behandeling (NFT). Na mediaan 5,9 jaar follow-up (range 0,5-10,0) was
de zes-jaars progressievrije overleving in subgroep A 91% met cRT versus 95%
met NFT (p=0,62); in subgroep B 98% met cRT versus 90% met NFT (p=0,24),; en in
subgroep C 89% met cRT versus 86% met NFT (p=0,53).
De
onderzoekers concluderen dat in patiënten met gevorderd HL met LNM en complete
metabole respons na twee en na zes cycli ABVD, cRT veilig achterwege kon worden
gelaten, ongeacht de grootte van de baseline-LNM.
