De Franse
gerandomiseerde fase 2-studie NeoPAL vergeleek de combinatie palbociclib plus
letrozol (LETPAL) met moderne chemotherapie als neoadjuvante behandeling voor
hoog-risico luminal mammacarcinoom. Dr. Paul Cottu (Institut Curie, Parijs) en
collega’s publiceren de studie online in Annals of Oncology.1
De studie includeerde 106 patiënten met ER-positief HER2-negatief
Prosigna-gedefineerd luminal B of luminal A en klierpositief stadium II of III
mammacarcinoom die initieel niet in aanmerking kwamen voor borstsparende
chirurgie. De mediane Prosigna ROR-score was 71 (range 22-93). De patiënten
werden gerandomiseerd naar negentien weken LETPAL of drie drieweekse cycli FEC
gevolgd door drie drieweekse cycli docetaxel.
Het primaire
eindpunt van de studie was percentage patiënten met residual cancer burden (RCB) 0 of 1. RCB 0-1 werd gezien in 7,7%
(95%-bti 0,4-14,9) van de patiënten in de LETPAL-arm versus 15,7% (95%-bti
5,7-25,7) van de patiënten in de chemotherapie-arm; pCR werd gezien in 3,8% met
LETPAL versus 5,9% met chemotherapie. Percentage patiënten met klinische respons
(75%) en borstsparende chirurgie (69%) verschilden niet significant tussen
beide armen. Preoperative Endocrine
Prognostic Index 0 scores waren 17,6% met LETPAL versus 8,0% met
chemotherapie. Het veiligheidsprofiel was als verwacht, met twee serious adverse events met LETPAL versus
zeventien (inclusief elf graad 4) SAEs met chemotherapie.
De
onderzoekers concluderen dat in beide armen de pathologisch complete respons
slecht was en de klinische respons bemoedigend. De biomarker-respons en
veiligheid waren aanzienlijk beter in de LETPAL-arm.

docetaxel as neoadjuvant therapy for high-risk luminal breast cancer. LETPAL
was associated with poor pathological response but encouraging clinical and
biomarker responses. Chemotherapy was associated with poor pathological and
biomarker responses, with a much less favorable safety profile than LETPAL.
