Oudere
patiënten met nieuw-gediagnostiseerd Philadelphia-chromosoom negatief ALL
hebben een slechte prognose. De combinatie van gerichte therapie met
lage-intensiteit chemotherapie is veilig en effectief. Een studie van MD
Anderson Cancer Center (Houston TX) vergeleek de combinatie van inotuzumab
ozogamicine plus lage-intensiteit chemotherapie (mini-HCVD) met of zonder
blinatumomab versus standaard intensieve chemotherapie (HCVAD) als frontline
behandeling voor Ph-negatief ALL in oudere patiënten. Dr. Elias Jabbour en collega’s publiceren de studie online in Cancer.1
De studie
includeerde 135 oudere patiënten met nieuw-gediagnostiseerd Ph-negatief ALL. Er
waren 77 patiënten die standaard HCVAD kregen en 58 patiënten die inotuzumab
ozogamicine plus mini-HCVD al of niet met blinatumomab kregen. De combinatie
van antilichaam met lage-intensiteit chemotherapie vergeleken met standaard
intensieve chemotherapie resulteerde in hoger percentage patiënten met respons
(98% versus 88%), lager percentage patiënten met vroeg overlijden (0% versus
8%), en lager percentage overlijden in complete remissie (5% versus 17%). In
propensity score analyse (38 patiënten in beide groepen) was de drie-jaars
gebeurtenisvrije overleving 64% versus 34% (p=0,003) en de drie-jaars overall survival 63% versus 34%
(p=0,004). In multivariate analyse was behandeling met inotuzumab ozogamicine
plus mini-HCVD met of zonder blinatumomab versus HCVAD geassocieerd met betere
OS (HR 0,550; p=0,020).
De onderzoekers
concluderen dat de combinatie van inotuzumab ozogamicine plus mini-HCVD met of
zonder blinatumomab een veilige en werkzame frontline behandeling was voor
Ph-negatief ALL in oudere patiënten, en geassocieerd was met betere uitkomsten
dan standaard HCVAD chemotherapie.
