GEMSTONE-302 werd uitgevoerd in 35 centra. De studie includeerde patiënten (leeftijd 18 tot en met 75 jaar; ECOG performance status 0-1) met niet-eerder behandeld stadium IV squameus of niet-squameus NSCLC zonder bekende EGFR-sensitiserende mutatie of ALK-, ROS1- of RET-fusies. De patiënten werden 2:1 gerandomiseerd naar intraveneus sugemalimab 1200 mg (n=320) of placebo (n=159) toegevoegd aan platina-gebaseerde chemotherapie. Het primaire eindpunt van de studie was progressievrije overleving.
Ten tijde van de finale PFS-analyse was de mediane follow-up 17,8 maanden. De mediane PFS was 9,0 maanden met sugemalimab versus 4,9 maanden met placebo (HR 0,48; p<0,0001). Treatment-related serious events werden gerapporteerd voor 23% van de patiënten in de sugemalimabgroep en 20% van de patiënten in de placebogroep. Behandelings-gerelateerd overlijden trof 3% van de patiënten in de sugemalimabgroep en 1% van de patiënten in de placebogroep.
De onderzoekers concluderen dat sugemalimab plus chemotherapie een nieuwe eerstelijns optie kan zijn voor zowel squameus als niet-squameus mNSCLC.

Summary: The multicenter phase 3 GEMSTONE-302 trial in China evaluated the PD-L1 inhibitor sugemalimab versus placebo, in combination with first-line platinum-based chemotherapy, for metastatic squamous or non-squamous NSCLC. The median progression-free survival was 9.0 months with sugemalimab versus 4.9 months with placebo.
