Ibrutinib monotherapie en de combinatie van chlorambucil plus obinutuzumab zijn beide standaard eerstelijns-behandelingen voor CLL. De multinationale fase 3-studie iLLUMINATE vergeleek ibrutinib plus obinutuzumab (IO) versus chlorambucil plus obinutuzumab (CO) voor niet-eerder behandeld CLL/SLL. iLLUMINATE wordt uitgevoerd in 74 centra in Australië, Canada, de Europese Unie, Israël, Nieuw-Zeeland, Rusland, Turkije, en de Verenigde Staten. De studie includeerde patiënten met niet-eerder behandeld CLL/SLL, in de leeftijd van 65 jaar of ouder, of jonger dan 65 jaar met co-existerende aandoeningen. Ze werden gerandomiseerd naar zes cycli IO (n=113) of CO (n=116). De randomisatie geschiedde gestratificeerd naar ECOG performance status en cytogenetische kenmerken. De patiënten en onderzoekers waren niet geblindeerd voor de studie-arm, maar het primaire eindpunt was progressievrije overleving centraal beoordeeld door een wel-geblindeerd review committee.

Na mediaan 31,3 maanden follow-up was de mediane PFS niet-bereikt in de IO-arm versus 19,0 maanden in de CO-arm (HR 0,23; p<0,001). De dertig-maands PFS was 79% met IO versus 31% met CO. De meest-gerapporteerde graad 3 en 4 adverse events in beide armen waren neutropenie en trombocytopenie. Ernstige AEs werden gezien in 58% van de IO-patiënten en 35% van de CO-patiënten. Behandelings-gerelateerd overlijden trof één patiënt in de IO-arm (plotse dood) en één patiënt in de CO-arm (neuroëndocrien huidcarcinoom).

De onderzoekers concluderen dat IO een werkzame en veilige chemotherapievrije behandeling is voor niet-eerder behandeld CLL en SLL, ongeacht hoog-risico kenmerken

Summary: The international phase 3 study iLLUMINATE compared first-line ibrutinib plus obinutuzumab versus chlorambucil plus obinutuzumab for CLL. After median follow-up of 31.3 months the median PFS was significantly longer in de ibrutinib-obinutuzumab arm than in the chlorambucil-obinutuzumab arm (not reached versus 19.0 months; HR 0.23; p<0,0001). Serious adverse events occurred in 58% of patients in the ibrutinib-obinutuzumab arm versus 35% of patients in the chlorambucil-obinutuzumab arm. Treatment-related death occurred in one patient in each arm.
