De studie randomiseerde patiënten met resectabel stadium II tot en met IIIB NSCLC naar vier cycli preoperatieve platina-gebaseerde chemotherapie plus durvalumab (n=400) of placebo (n=402) iedere drie weken; na de chirurgie gevolgd door twaalf cycli durvalumab of placebo iedere vier weken. Primaire eindpunten was gebeurtenisvrije overleving (gedefinieerd als tijd tot progressieve ziekte die chirurgie onmogelijk maakte, recidief, of overlijden aan any cause) en pathologisch complete respons. De duur van EFS was significant langer met durvalumab dan met placebo (HR 0,68; p=0,004). Na twaalf maanden follow-up werd EFS gezien in 73,4% van de durvalumabgroep versus 64,5% van de placebogroep. De incidentie van pathologisch complete respons was 17,2% in de durvalumabgroep versus 4,3% in de placebogroep (p<0,001). Graad 3 of 4 adverse events werden gezien in 42,4% van de durvalumabgroep en 43,2% van de placebogroep.
De onderzoekers concluderen dat onder patiënten met resectabel NSCLC, perioperatief durvalumab plus neoadjuvante chemotherapie resulteerde in betere EFS en pCR dan alleen neoadjuvante chemotherapie.

Summary: The multinational phase 3 AEGEAN trial found that in patients with resectable NSCLC, perioperative durvalumab plus neoadjuvant chemotherapy was associated with significantly greater event-free survival and pathological complete response than neoadjuvant chemotherapy alone.
