De analyse includeerde 82 patiënten van NKI (OpACIN 9 en OpACIN-neo 33), Melanoma Institute Australia (OpACIN-neo 38), en Karolinska Universiteitsziekenhuis (OpACIN-neo 2) die zes weken neoadjuvant ipilimumab plus nivolumab kregen voor hoog-risico stadium III melanoom, gevolgd door TLND. Het eindpunt van de analyse was concordantie tussen pathologische respons in ILN en pathologische respons in het gehele TNB. Deze concordantie werd gezien in 81 van 82 patiënten (99%); in de ene discordante patiënt werd de TNB-respons onderschat op basis van de ILN-respons. Concordantie tussen ILN-respons en respons in elke afzonderlijke klier werd gezien in 79 van 82 patiënten (96%). Twee patiënten hadden ieder één kleine nonindex-klier met pathologische nonrespons, terwijl alle overige klieren, inclusief de ILN, partiële respons hadden. Het is mogelijk dat achterwege laten van TLND in deze twee patiënten het risico van regionaal recidief zou hebben verhoogd.
De onderzoekers concluderen dat de analyse suggereert dat pathologische respons van de ILN kan worden beschouwd als een betrouwbare indicator van respons van het gehele TNB. Een prospectieve studie van ILN-respons geleid achterwege laten van TLND is gewenst.

Summary: Retrospective analysis of two prospective studies found that in stage III melanoma pathologic response of the index lymph node to neoadjuvant ICI therapy may be considered a reliable indicator of the entire therapeutic lymph node dissection specimen response and may support the ILN response-directed omission of TLND in a prospective trial.
