Tussen begin 2010 en eind 2017 ondergingen 77 oudere patiënten in het ziekenhuis van de universiteit transorale robotische chirurgie voor HPV-geassocieerd OPC. De mediane leeftijd van de 58 mannen en 19 vrouwen was 73,0 jaar (IQR 71,0-76,0). De perioperatieve mortaliteit was 1,3% en orofarynx-bloeding werd gezien in 2,6%. Zevenentwintig patiënten (35,1%) kregen postoperatieve radiotherapie en 20 patiënten (26,0%) ondergingen postoperatieve chemoradiotherapie. Met mediaan 39,6 maanden follow-up (range 0,2-96,2) was na drie jaar de ziekte-specieke overleving 92,4%; de overall survival 90,0%, en de ziektevrije overleving 84,3%.
De onderzoekers concluderen dat transorale robotische chirurgie en aan pathologische kenmerken aangepaste adjuvante therapie voor HPV-geassocieerd OPC in oudere patiënten kan resulteren in gunstige overlevingsuitkomsten, infrequente perioperatieve mortaliteit, en in geselecteerde patiënten in de mogelijkheid chemotherapie achterwege te laten.
