De studie includeerde 141 mannen en 91 vrouwen die in de leeftijd van 75 jaar of ouder tussen begin 2006 en eind 2014 chirurgie ondergingen voor stadium 1 longcarcinoom. Er waren 156 patiënten die lobectomie ondergingen en 76 die SR ondergingen (n= 50 segmentectomie, n=26 wedge resectie). Leeftijd, tumorgrootte, tumorlocatie, en Adult Comorbidity Evaluation 27 (ACE-27) waren significant van invloed op de keus voor SR. Geen van de patiënten overleed als gevolg van de ingreep. De incidentie van graad 3b of hoger postoperatieve complicaties was 5,1% na lobectomie versus 5,3% na SR. In propensity-score gematchte analyse (n=57, tumorgrootte 23 mm, consolidatie/tumor ratio 83%) was de vijf-jaars overleving 81,1% na lobectomie versus 73,5% na SR (p=0,4374). De preoperatieve ACE-27 score was een sterkere prognostische factor dan type chirurgie .
De onderzoekers concluderen dat de ernst van preoperatieve comorbiteiten een significante prognostische factor was, en dat SR als compromis chirurgische procedure veelbelovende korte- en lange-termijn uitkomsten had in geselecteerde geriatrische patiënten met stadium 1 longcarcinoom.

Summary: A retrospective study in Kyoto (Japan) found that in high-risk operable geriatric patients undergoing surgical resection for stage I lung cancer, the severity of preoperative comorbidities was a significant prognostic factor. Sublobar resection as a compromise surgical procedure may provide promising short- and long-term outcomes in this population.
