KEYNOTE-189 includeerde 410 patiënten in de pembrolizumab-combinatiegroep en 206 patiënten in de placebo-combinatiegroep. Zowel de PFS (HR 0,50; 95%-bti 0,42-0,60) als de OS (0,60; 0,50-0,72) waren significant langer in de pembrolizumab-combinatiegroep dan in de placebo-combinatiegroep, met vijf-jaars OS-percentages 19,4% respectievelijk 11,3%. De figuur laat zien dat het overlevingsvoordeel met pembrolizumab werd gezien in alle onderscheiden subgroepen. Onder de 57 patiënten die 35 cycli pembrolizumab voltooiden was het drie-jaars OS percentage na voltooiing 71,9%.
De onderzoekers concluderen dat deze data gebruik van pembrolizumab plus pemetrexed-platina als eerstelijns therapie voor metastatisch nsqNSCLC zonder EGFR/ALK-veranderingen steunen.

Summary: The multinational phase 3 KEYNOTE-189 trial randomized patients with previously untreated metatstatic nonsquamous non-small cell lung cancer,without EGFR/ALK alterations, to pembrolizumab or placebo, both added to platinum-pemetrexed chemotherapy. Updated results of the study, after median 64.6 months follow-up (range 60.1-72.4) show that pembrolizumab plus pemetrexed-platinum maintained OS and PFS benefit versus placebo plus pemetrexed –platinum.
