De studie includeerde patiënten met resectabele CRC liver-only metastasen. De patiënten kregen preoperatief één dosis tremelimumab plus durvalumab, en postoperatief durvalumab monotherapie. Onder de 23 geïncludeerde patiënten (21 pMMR, 2 dMMR) waren er 17 (74%) die chirurgische resectie ondergingen. Graad 3 of 4 behandelingsgerelateerde immuuntoxiciteit werd gezien in 5 van 23 patiënten (22%) en postoperatieve graad 3 of 4 toxiciteit in 2 van 17 patiënten (12%). Er waren vier patiënten met pathologisch complete respons (beide dMMR-patiënten en twee patiënten met POLE-mutatie). Er waren aanwijzingen voor activering van T-cellen en B-cellen. De mediane recidiefvrije overleving was 9,7 maanden (95%-bti 8,1-17,8) en de mediane overall survival was 24,5 maanden (95%-bti 16,5-28,4).
De onderzoekers concluderen dat de neoadjuvant combinatie van tremelimumab en durvalumab voorafgaand aan CLM-resectie veilig was en mogelijk resulteerde in T- en B-celactivering.

Summary: A pilot study at MD Anderson Cancer Center (Houston, TX) evaluated perioperative anti-CTLA4 and anti-PDL1 therapy for resectable CRC liver metastases. Patients received one dose of tremelimumab plus durvalumab preoperatively and single-agent durvalumab postoperatively. Four of 24 patients had pathologic complete response. The median RFS was 9.7 months (95%CI 8.1-17.8) and the median OS was 24.5 months (95% CI 16.5-28.4).
