Ondanks de
introductie van nieuwe screeningsprogramma’s, behandelingen, en
monitoringstechnieken in patiënten met mammacarcinoom is er behoefte aan
bruikbare testen voor het monitoren van respons op behandeling en als basis
voor klinische beslissingen. Een multicenterstudie in het Verenigd Koninkrijk
heeft de waarde onderzocht van bepalen van niveaus van plasma-celvrij DNA
(cfDNA) en circulerende tumorcellen (CTCs) als voorspellers van uitkomsten van
metastatisch mammacarcinoom (MBC). Prof. Jacqueline Shaw (University of Leicester) en collega’s publiceren
de studie online in Breast Cancer Research.1
De studie
includeerde 194 patiënten met radiologisch bevestigd MBC. cfDNA-niveaus werden
bepaald met qPCR; en CTC-counts met CELLSEARCH. In multivariate analyse waren
zowel het cfDNA-gehalte als aantal CTC’s voorspellers van overall survival. Gehalten van cfDNA waren ook voorspellend voor
progressievrije overleving en
ziekterespons. Gecombineerde analyse van CTCs en cfDNA was meer informatief voor het voorspellen van OS dan
combinatie van de conventionele biomarkers CA15-3 en AP.
De
onderzoekers concluderen dat cfDNA-niveaus en CTC-gehalten prognostische
biomarkers zijn in patiënten met MBC.
