Onder
patiënten met mammarcarcinoom is een hoge tumor/stroma-ratio (TSR) geassocieerd
met betere prognose. Een post-hoc analyse van de prospectieve DIRECT- en
NEOZOTAC-studies heeft de associatie van TSR met Miller-Payne (MP)-score en
pathologische respons van HER2-negatief mammacarcinoom op neoadjuvante
chemotherapie onderzocht. Dr. Wilma Mesker (LUMC) en collega’s publiceren de analyse in het International Journal of
Cancer.1
De
onderzoekers bepaalden de TSR in 375 H&E gekleurde secties van
pre-treatment biopten. Stroma-lage tumor was significant geassocieerd met
hogere MP-score (p=0,005). Deze relatie bleef significant in de ER-negatieve
subgroep (p=0,047). Stroma-lage tumor was in univariate analyse geassocieerd
met pCR (OR 2,46; p=0,004). Deze relatie werd niet-significant in multivariate
analyse (OR 1,90; p=0,119). In subgroep-analyse was de univariate OR 5,91
(p=0,030) voor ER-negatieve tumoren en 1,48 (p=0,281) voor ER-positieve
tumoren.
De
onderzoekers concluderen dat hoge TSR geassocieerd was met hogere MP-score en
hoger pCR-percentage.

Summary: Post-hoc analysis of the prospective DIRECT and NEOZOTAC trials showed that
among patients with HER2-negative early-stage breast cancer a high pretreatment
tumor/stroma-ratio was associated with a higher Miller-Payne score an higher
pCR rate to neoadjuvant chemotherapy.
